These activities seek to improved the quality and delivery of services to people with developmental disabilities and their families. Individual and Family Survey - You should take this survey if you are a New York State resident with a developmental disability or a family member of a New York State resident with a developmental disability. Provider Survey - You should take this survey if you are a New York State organization or individual providing services for persons with developmental disabilities and/or their families. Consumer Satisfaction Survey - You should take this survey if you have received services or supports within the last three years through a any program funded in whole or in part by the New York State Developmental Disabilities Planning Council. You should also take the Individual and Family Survey. Grantee Follow-Up Survey - You should take this survey if you have received an email or letter correspondence from the New York State Developmental Disabilities Planning Council instructing you to do so.
