The New York State Developmental Disabilities Planning Council is seeking information from organizations and individuals
providing services and supports to children and adults with developmental disabilities and/or their families.

The DDPC demonstrates new approaches to providing services, supports and other assistance to persons with
developmental disabilities and/or their families. The information that we are soliciting will be used to help the DDPC
continue to develop and demonstrate new and innovative ways of making life better for New Yorkers with developmental
disabilities and their families.
