| Survey Description: | From its inception, the DDPC has been a firm supporter of the Self-Determination program in NYS and the belief that individuals should direct their lives as much as possible. The DDPC has periodically asked self-advocates and their parents/family members/guardians to critically assess the progress of the Self-Determination program and, more generally, individual’s ability to be involved in the decisions that affect their lives. To this end, we have designed the following survey. If you are involved in the Self-Determination Program (CSS) or have comments concerning individual choice, please take a moment to consider these questions. |
